Acknowledgment of GREAT Training and Gluten-Free Intent

FOODSERVICE: I acknowledge that I will complete the GREAT training program in its entirety, including reading the GREAT Manual and viewing the online visual presentation. I agree to incorporate this training into my services, including preparation/handling of gluten-free food, training of others on gluten-free protocol, and/or reviewing procedures for gluten-free safety. I am aware that people with celiac disease and gluten intolerance cannot safely consume food containing gluten that is found in wheat, rye and barley and their derivatives. By taking this training, I agree to develop and/or assist in developing a gluten-free protocol for preparing and delivering gluten-free food that is free of cross contamination and understand that this can occur at any stage of the food handling, preparation and delivery cycle. I acknowledge that I cannot and will not hold NFCA responsible for any sickness caused by gluten-free food which is not properly and safely prepared and delivered.

I agree to incorporate this training into my services, including the care of those with celiac disease. I am aware that people with celiac disease and gluten intolerance cannot safely consume food containing gluten that is found in wheat, rye and barley and their derivatives. I acknowledge that this course is not intended nor suited to be a replacement for professional medical advice. I acknowledge that I cannot and will not hold NFCA responsible for any sickness, health issues or any other complaints made by those in my care.
